INSURANCE & PAYMENTS
Insurance:
We DO NOT accept insurance plans. Your annual fee is for access to our services all day, every day and preventive care that is not covered by insurance. Patients will need to maintain a HSA, PPO, POS, Medicare or other major medical plan to cover services provided outside of our office, such as referrals and access to specialists, hospitalization, emergency room care, laboratory work, radiology imaging, and prescriptions.

Additional Fees:
There is an additional $25 dollar fee for completion of forms for use outside of the patient's medical record (forms have a 72 hr turn around time), including:
